Questions a patient may ask about the Membership Plan

Question: Can I purchase the In House Membership Plan when I max out of my Benefit Plan?
Answer: No, the patient will still receive a discount from us through their benefit plan

Question: My Benefit Plan only covers Preventative and Diagnostic, can I purchase the
Membership Plan to cover the costs of treatment outside of preventive & diagnostic?
Answer: Yes,

Question: My Benefit Plan Covers everything but Implants, can I purchase the Membership
Plan to cover the cost of Implants
Answer: No

Question: I’ve had the In House Membership Plan for 6 months and now just purchased a
Benefit Plan, can I cancel the Membership Plan now, or wait until their year is up?
Answer:We will review your billing and see what benefits you’ve already used and then will
decide.

Question: I’m a Perio Patient, How many cleanings do I get per year?
Answer: With our In House Membership Plan, you will receive two Periodontal Maintenances
with our plan and then any additional Periodontal Maintenance would be 20% off.
